Keep a Symptom Diary
Log each symptom's severity as it changes, your pain on a 0 to 10 scale, and bowel movements with their stool form, on the Symptoms page.
Updated
Health › Symptoms is a symptom diary. Each entry records one moment, so the diary shows how a symptom started, changed, and ended.
Log a Symptom
- Choose Log, then Symptom.
- Choose the Symptom, such as a headache, nausea, or a cough.
- Choose its Severity: None, Mild, Moderate, Severe, or Very severe, the scale of the US National Cancer Institute’s PRO-CTCAE, used in clinical studies.
- Set the time and choose Log.
When it gets better or worse, log it again with its new severity. When it’s gone, log it as None: that’s what ends it in your diary.
Log Pain
Pain intensity is logged on its own, on the standard numeric rating scale from 0 (no pain) to 10 (the worst pain you can imagine). Choose Log, then Pain Intensity.
Log a Bowel Movement
- Choose Log, then Bowel Movement.
- Choose its Bristol Stool Form, from type 1 (separate hard lumps) to type 7 (entirely liquid). Each type is described in the list.
- Tick what applies, as bowel diaries for IBS ask: Urgency, Straining, Incomplete Evacuation, Anorectal Blockage, Manual Maneuvers, Mucus in Stool, or Blood in Stool.
- Choose Log.
What the Page Shows
How severe your symptoms were, your pain, and your stool form over time; then every symptom, pain, and bowel movement you logged, newest first. Symptoms from an Apple Health export come in with their severity too.
To see what goes with a symptom, such as headaches after short nights, compare metrics on one chart.
